Collaboration for Older Adult Safety – September CARE Connections

In the U.S., more than 20 million older people visit emergency departments each year. What happens next can change their lives.

Read how the School of Nursing is contributing to research to make care transitions safer for older adults in the September issue of CARE Connections, the monthly newsletter of the SoN’s Center for Aging Research and Education. Here’s an excerpt:

“From the emergency department, older adults might get admitted to the hospital, Or they might go to a skilled nursing facility. Or they might go home. Each of those transitions requires different information and types of communication.”
