This 0.8 position will float between PMs (3p-11p) and Nights (11p-7a); four shifts per week and a weekend and holiday rotation.

Unity is a 501(c)(3) nonprofit community provider of hospice care, treatment plus and grief support in 13 counties throughout Northeast Wisconsin. A true leader in its field, Unity has the distinction of having opened the state’s first hospice in 1977, the state’s first non-hospital based palliative care program in 2002 and the area’s first hospice residence in 2007. We provide a full spectrum of end-of-life care and education benefiting all members of the community.
Position Summary
Responsible and accountable for providing quality professional nursing care to patients and their families adhering to the WI Nurse Practice Act, Administrative Code of the WI Board of Nursing, and/or appropriate nursing standards governing nursing in the state of practice, the ANA Nursing Scope and Standards of Practice, the ANA Code of Ethics for Nurses and Unity Policies and Procedures. The responsibility includes providing a safe environment for patients, families, staff, and visitors and working collaboratively with the multi-disciplinary care team to provide the best outcomes for the patient and family. Community involvement and collaboration is encouraged.
Key Job Duties
Crisis Registered Nurse
•Provide skilled nursing assessment and appropriate intervention
•Update the plan of care as necessary
•Respond to urgent patient/family needs in collaboration with triage nurse
•Initiate crisis care as needed
•Report appropriate findings and significant changes to the primary team, attending physician and primary caregiver
•Provide education to patient/family members on disease progression, symptom management, medication and other associated patient care needs
•Administer and titrate medication according to physician/nurse practitioner orders
•Record all received orders to the Electronic Medical Record and initiate follow up as needed
•Prioritize daily work and make adjustments taking into account the needs of patients, Unity and other staff
Admissions Registered Nurse
•Review information concerning the patient’s medical background
•Identify the appropriate decision maker/agent and follow through with Durable Power of Attorney for Healthcare (DPOAHC), activation and “In Lieu Of” forms
•Provide an explanation of hospice and palliative programs and answer questions appropriately
•Review and complete admission forms with the patient/family
•Complete a comprehensive nursing assessment which includes medication reconciliation and obtain orders as needed
•Collaborate with members of the medical team to determine appropriate services as needed
•Notify referral sources and attending physicians of admission to the hospice or palliative program
•Assess need for and order supplies and durable medical equipment
•Enter appropriate information into the electronic medical record and initiate follow up
•Communicate with the primary team and support services to meet the needs of the patient
•Collaborate with the Director of Patient Services and other departments as appropriate on process improvement efforts
•Participate in focus groups or other specialized work groups
Meng Residence Registered Nurse
•Provide clinical and supporting case management based on patient’s diagnosis and end-of-life symptoms
•Complete initial, comprehensive and updated assessments of patient and family needs
•Develop, implement and review care plans for patient and family. Update care plan as the needs of the patient and family change
•Prepare for and participate in interdisciplinary team meetings as scheduled
•Communicate with all interdisciplinary team members
•Report appropriate findings and communicate significant changes to attending physician and process physician/NP orders
•Manage individual patient medication and Residence contingency medications
•Administer and titrate medication according to physician/nurse practitioner orders
•Delegate nursing duties to LPN and supervise care provided by CNA
•Prioritize daily work and make adjustments while taking into account the needs of patients, Unity and other staff
•Complete new patient admissions as needed
