RN Case Manager Opportunity within a growing Home Care office!
This position works with our Director of Nursing and Branch Manager out of our well-established BrightStar of Madison office and is responsible for providing care management services in a home care environment. The Case Manager is responsible for overseeing the care provided to our clients as directed by BrightStar Care and our Joint Commission accreditation quality and safety guidelines. A typical day may include performing new client assessments, overseeing the care of existing clients, medication reconciliation, wound care, and care coordination with skilled service organizations and hospices. Compassion, patience and an outgoing personality are qualities that will build trust and a level of comfort with families. The Case Manager will work primarily in the field and rarely spend time in the main office. Travel in Dane County required.
Case Manager Essential Duties and Responsibilities:
- Provides care as defined within the scope of the state’s nurse practice act
- Makes the initial evaluation visit and regularly reevaluates the client’s nursing and/or personal assistance needs
- Assists with the intake of new clients and informs potential clients of our services and differentiator
- Provides those services requiring substantial and specialized nursing skill
- Initiates appropriate preventive and rehabilitative nursing procedures and counsels client and family on nursing related needs
- Prepares clinical and progress notes and summaries of care
- Assists in the development of organizational goals and agency performance improvement activities
- Administers agency policies and procedures
- Assists with assessing the competency of staff to promote quality, continuity and safety of client care
- Provides information to be used for statistics, reports and records for purposes of evaluation and reporting of organizational activities
- Assures proper maintenance of clinical records in compliance with local, state and federal laws
- Promotes home health referrals in the health care community
Position Requirements
Education and Experience:
- Registered nurse with current unencumbered license to practice professional nursing in Wisconsin
- Minimum of one (1) year of documented experience required. Two (2) or more years of clinical experience with one (1) year experience in a home health agency, primary care clinic or health facility, preferred
- Flexibility and good time management skills A MUST
- Possess exceptional nursing assessment skills
- Experience with, or desire to, participate in marketing and sales a plus
- Comply with accepted professional standards of practice
- Excellent interpersonal skills
- Analytical/Critical thinker and problem solver
- Excellent customer service skills
- Treat clients, staff and the public with courtesy, respect and presents a positive public image
- Case management skills a plus
- Ability to travel including weekends, nights and holidays if necessary
- Understands regulatory compliance such as OSHA and HIPAA
